- Adult Day Care - Supervised day care and health-related services are provided in a licensed facility by professional staff. Clients are cared for in a group setting by the hour or by the day.

- Case Management -The case manager is considered the “gatekeeper” for most services by determining the clients needs and eligibility. The case manager for all homebound clients performs a complete physical and cognitive assessment and develops a care plan for the individual. Unmet needs are identified and referrals to appropriate services are made. The client’s needs are re-evaluated on a regular basis and the services provided are updated at that time.
Specifically related to the Family Caregiver Program, this service is to be provided to caregivers of persons age 60 years or older and grandparents or older relative caregivers of children (under 19 years of age). Caregivers are considered to be the client. For other Home and Community Based Services, the person in need of care is considered the client.

- Congregate Meals - A service that provides for a nutritious meal containing at least 1/3 of the Recommended Dietary Allowance for an individual in a congregate setting. Nutrition education and social interaction are provided.

- Family Caregiver Programs & Services - The enactment of the Older Americans Act Amendments of 2000 established an important new program, the National Family Caregiver Support Program (NFCSP). This initiative allocates funding to Area Agencies on Aging for programs and services to support family caregivers. Programs and services provided may include respite care, home repair and adaptation, training, legal assistance, assistive devices, support groups for grandparents raising grandchildren, and information and referral.

- Home Delivered Meals - service that provides for a nutritious meal containing at least 1/3 of the Recommended Dietary Allowance for an individual, delivered to his/her place of residence.

- Home Health Aid - Provides intermittent treatment, monitoring and/or maintenance of a health condition and supportive care for activities of daily living at the individual’s place of residence. This may include, but is not limited to, monitoring vital signs, changing dressings, assisting with self-medications, providing skin care and foot care, providing bathing, shampooing, shaving and toileting, all under the supervision of a licensed Registered Nurse.

- Housekeeping - A service that provides assistance in the performance of routine household activities at an individual’s place of residence. Housekeeping is usually done on a once-a-week basis. Heavy cleaning, yard work, laundry and shopping services may also be provided.

- Legal Services - This service is provided to clients who meet eligibility requirements and have no resources to employ an attorney. Volunteer legal advocates can provide certain aspects of legal assistance.

- Medicare/Benefits Counseling - Trained counselors assist with questions about Medicare, Medigap insurance and other public benefits.

- Ombudsman Program - Volunteers are trained to visit residents in long-term care facilities to ensure that quality care is being received. Ombudsmen investigate and attempt to resolve complaints, and act as advocates for residents.

- Personal Care - Provides assistance with personal physical needs such as assistance with showering, bathing, toileting, dressing and shampooing. Assistance with tasks necessary for the comfort and safety of the movement-restricted or other tasks may be provided when no medical or nursing supervision is required.

- Respite Care - A service that provides short-term care and supervision consistent with the health needs of the person, to supplement existing care, to provide a safe living environment, and to support or relieve the burden of caregivers.

- Transportation - This service is available to seniors who have no transportation for medical appointments, shopping, paying bills, or attending the meals programs. Transportation may be offered by project-operated vans or by other means.

- Visiting Nurse - A service that provides nursing intervention that may include patient care, coordination, facilitation and education. Skilled nursing services may be provided.
